I hereby absolve TAPA from any liability in the event of any injury to my child. I give TAPA permission to seek medical attention at the time of accident or serious illness.
I give permission for my child to be photographed and/or videoed whilst participating in TAPA activities and I consent to these photos/videos being used for publicity purposes.
